Thank you for your interest in Special Smiles!

Below is the process for becoming a new patient at Special Smiles. This also applies if you are a returning patient that has not been seen for at least 5 years. If you have any questions about the processfeel free to contact us at 267-639-6250 prompt #2.

We want to ensure your visit goes smoothly. Here’s how you can prepare so we can be ready for you!

 

STEP 1

Information Gathering

  • Obtain a Referral: Get a referral from a dentist for treatment under general anesthesia OR a Letter of Necessity from your Primary Care Physician.
  • Photo of Insurance: Capture clear pictures of both sides of your insurance card.
  • Supportive Housing: For any individual living in supportive housingwe require a copy of the Lifetime Medical History or what is sometimes referred to as an Individualized Service Plan (ISP).

If you have recent X-raysfeel free to upload them when filling out your Health History Form—they are helpfulthough not required.

Once you have these documents preparedyou may fill out the Health History Form.

STEP 2

Scheduling Your Consultation

Once we have your infowe’ll contact you within seven business days to schedule your consultation. You’ll meet with our nurse and dentist to discuss the specific medical clearances needed prior to your dental care under general anesthesia in our outpatient office.

Note: This is a planning meeting; your actual dental care happens at a later date.

Our professional staff will review all of the information to determine the best treatment plan.

If we determine Special Smiles is the best place for the patient’s dental carewe will begin the process for scheduling your dental treatment appointment.

If Special Smiles is not the best place for the patient’s dental carewe will do our best to assist you in finding an appropriate dental facility.

STEP 3

Obtain Required Medical Clearances

Following your consultationyou will need to gather the following to ensure it’s safe to proceed. To make it easyyou will leave your consultation with the History & Physical and Consent Forms in hand:

  • History & Physical: A current H&P and medication list from your PCP (using the form we provide).
  • Legal Consents: Signed consents from the patient’s legal guardian.
  • Specialist Clearances: Depending on your historywe may need office visit notes and clearance from your specialists and/or instructions on holding medications.

Important: Please email or fax these completed forms to us as soon as possible. Once everything is received and reviewed by our medical teamour scheduling coordinator will call you to schedule the dental treatment date!

STEP 4

Day of Treatment

On treatment dayyour dentist and anesthesiologist will meet with you to review the plan and answer any questions.

  • Time: Plan to be at our office for approximately 4 hours.

The dental treatment consists of an examination; full mouth x-rayscleaningapplication of fluorideand other dental services depending on the needs of the patient.

Following treatmentthe patient is awakened and transferred to the recovery roomwhere our nursing staff provides one-on-one care until the patient is able to return home.

  • Aftercare: The patient will need dedicated attention at home to ensure they recover from anesthesia as expected.
